We are excited that you would like your child to become our patient!

The first step to transferring your child to our office is to complete a medical record release so that we can retrieve your child’s records from your previous doctor. The practice functions most effectively as a medical home if patients/families provide a complete medical history and information about care obtained outside the practice.
